For enrollment, please print the forms below and return them to the school prior to the planned start date. Our school and the State of Texas require an Updated Immunization Record for your child and a Physician Statement which shows that your child is healthy and may participate in the school program. This Physicians’ statement must be submitted within 2 weeks of your child’s start date and be kept in the student’s file.
